"Bug Your Doc - Get 3 Shots!" Materials


Digital versions of the "Bug Your Doc" educational materials in English and Spanish are available for free upon request. Doctor's offices, health departments, and other organizations can use these materials with parents and teens to spread the word about the importance of adolescent vaccines.

If your organization is interested in using any of the "Bug Your Doc"

education materials:

  1. Download, sign and scan the License Agreement form
  2. Open the online Request Use of Materials form.
  3. Upload the License Agreement into the online request form
  4. Complete and submit the online request form


Upon approval of your request, digital files of the requested materials will be sent to you. Your organization is responsible for printing any requested materials.  Unfortunately, our organization does not have funding to provide printed materials.
